Applying for services is easy!

Call our trained professionals at 617-292-6211. Our staff will evaluate what home care services you (or the person you refer) may need. You can also:
- Mail or fax us a letter
- Use our on-line Referral Form
- E-mail your referrals or any questions to info@elderinfo.org


Once your information is received by a professional Intake Care Manager, our staff will:
- Arrange a visit to your home
- Describe our services in depth, and
- Discuss with you the services that best meet your needs


The care manager will set up your services. If we can't provide the help you need, we will refer you to the right resource.

It is important that your services remain helpful to you as your needs change. We will visit you, call you throughout the year and make sure that you are receiving the right kind and amount of help.

Call your Care Manager with any questions. Your Care Manager will make sure that you receive the best services possible.
