Care Management
Activities related to a comprehensive client-oriented Needs Assessment consisting of
identifying current client unmet needs or problems that inhibit independent living within
the community; assisting the client in one or more home care services; and developing and
initiating a home care service plan for the client.
Case management also includes monitoring and evaluating the client's home care plan in
terms of its continued effectiveness in maintaining independence.
