Did you know? HomePower is a joint venture between Boston Senior Home Care and the Visiting Nurse Association of Boston, two of Greater Boston’s longest serving providers of in-home care. HomePower has been in existence for over ten years.

What is HomePower?
HomePower is a Group Adult Foster Care program funded by MassHealth that provides daily personal care and some homemaking to clients who have functional and/or cognitive impairment which warrants consistent personal care/homemaking to remain safe and independent in the community. Additional services such as Home Delivered Meals, Personal Emergency Response (PERS), Adult Day Health, Laundry and Grocery Shopping etc. can also be provided when needed.

Eligible candidates must meet the following criteria:
- Be at least 22 years old
- Have MassHealth (standard)
- Live in a building versus home
- Clients may be chronically ill, they do not need to be homebound.

Once admitted to the program:
The HomePower Nurse and Case Manager make alternate monthly home visits for case management and oversight. The personal care homemaker visits client daily to assist with personal care homemaking according to the nursing developed care plan.

So, who can make the referral?
