Nursing Screenings Registered Nurses screen for clinical eligibility for Medicaid reimbursement of health services such as Skilled Nursing Facility, Home Health Care, Adult Day Health, PACE, Personal Emergency Response System, Medicaid Waiver, etc. Nurses visit clients, provide Medical Consultation with Case Managers, develop Personal Care plans, provide consultation for Occupational and Physical Therapy and Nutrition Referrals, and recommend alternatives to nursing home placement.
